Conduct psychosocial assessments of the patient and family

Chaplains Who
Provide spiritual care for the patients and families
Assesse the spiritual needs of the patient and encourages a Review of Life

Bereavement Services
Telephone calls and cards for one year following the death. The purpose is to provide timely emotional and empathetic support

Annual Memorial Service. The purpose of this service is to give family members and staff an opportunity to celebrate the lives and honor the memories of loved ones who died on Hospice service
