In-Patient Services
- Short term inpatient care may sometimes be necessary for
symptom management during a medical crisis
- Hospice of Yuma is contracted with a local skilled nursing facility to partner in providing this care when needed
Hospice Offers a Full Time Medical Director Who
- Specializes in palliative care
- Makes home visits
- Works with the patient's primary physician in delivering care
Nurses Who
- Specialize in pain and symptom management
- Teach the expected course of the disease, care techniques and other health measures to the patient and family
- Provide counseling and supportive care to the patient and family
- Work with the patient's primary physician and other hospice team members to coordinate the plan of care
- Monitors the patient's condition and reports to the primary physician
- Are available to the patient and family 24 hours a day, seven days a week
Certified Nursing Assistants Who
- Are specially trained to care for hospice patients and are sympathetic toward the care of the sick, helping to lessen the stress on the caregiver
- Provide bathing assistance
- Provide personal hygiene and nail care
- Assist with activities of daily living
- Teach relaxation techniques
- Provide respite for the caregiver
Homemakers Who
- Provide light housekeeping services, if needed. Activities may include laundering the patient's linen and clothes, vacuuming, dusting, washing dishes, cleaning the bathroom, assessing safety in the home and providing companionship
Social Workers Who
- Collaborate with other hospice team members to provide holistic care to patients and their loved ones
- Conduct psychosocial assessments of the patient and family
- Develop and update plans of care to meet the patient/family needs
- Facilitate communication between patients and loved ones
- Use intervention techniques for improved coping and problem-solving skills
- Link patients/families with community resources
Chaplains Who
- Provides spiritual care for the patients and families
- Assesses the spiritual needs of the patient and encourages a Review of Life
- Arranges for different community clergy to attend the patient's needs upon request
- Arranges for rituals upon request
- Conducts a personalized memorial or funeral when requested by the family
Bereavement Services
- Telephone calls and cards for one year following the death. The purpose is to provide timely emotional and empathetic support
- Home or office visits at the request of bereaved family members. The purpose of these visits is to offer compassionate listening, caring and direction
- Self-help through materials and referrals
- Annual Memorial Service. The purpose of this service is to give family members and staff an opportunity to celebrate the lives and honor the memories of loved ones who died on Hospice service
- Scheduled, group support, both educational and social
Palliative Care Services
- Physician consults for the hospital, assisted living, extended care facilities & home settings
- A referral from a primary care physician is required
